Thorough Assessments: Conducting detailed assessments over the phone to evaluate if the client is a good fit for our services and to identify their specific needs. Service Determination: Determining the appropriate services required based on assessment outcomes and client needs. Client Connection: Ensuring clients are connected to the appropriate staff and services necessary to meet their needs. Seamless Referral Collaboration: Collaborating with other staff and community service providers to ensure seamless referral services.
